 men suffer from at different stages of their lives, and although it is not a topic that many people prefer to talk about, sexual dysfunction is a common health issue that affects a person's psychological, emotional and social life.

Defined as the inability to achieve or maintain an erection sufficient to perform sexual activity, ED can be caused by a combination of psychological and physical factors and is considered a health issue that significantly affects a person's quality of life. According to the World Health Organization (WHO), ED is a common health issue affecting about 30-40% of men over the age of 40.

ED can be a temporary condition, such as in cases of stress or anxiety, or it can become a chronic issue that requires ongoing treatment, and the causes of ED vary between psychological factors such as anxiety and depression, and physical factors such as chronic diseases or taking certain medications.

 

Causes of ED

The causes of ED are many and varied. They can be categorized into:

Physical causes:

  1. Cardiovascular disease: Issues such as high blood pressure or diabetes can affect blood flow to the genitals, leading to erectile dysfunction, according to a study published by the National Heart, Lung, and Blood Institute.
  2. Medications: Certain medications such as antidepressants, blood pressure medications, and cancer medications may affect sexual function.
  3. Smoking and alcohol use: Both narrow blood vessels, affecting blood flow to the penis.
  4. Hormones: Low testosterone levels can lead to low libido and erectile dysfunction, as shown in National Center for Biotechnology Information (NCBI) studies.

Psychological causes:

  1. Anxiety and stress: Many men experience anxiety during sexual performance, which can lead to temporary erectile dysfunction.
  2. Depression: Depression can affect sex drive and the ability to get an erection.
  3. Relationship issues: Tensions and unhealthy relationships can affect your sex life.

Psychological and emotional causes:

  1. Emotional stress: Daily life issues such as work, family, and money can worsen libido.
  2. Guilt or shame: Self-doubt or emotional distress may lead to avoidance of sexual activity.

 

How to diagnose

To diagnose ED, the doctor usually reviews the patient's complete medical history, including a physical examination to check for any health issues that may be causing the issue, and the doctor may also order some laboratory tests such as:

  • Blood tests to measure hormone levels such as testosterone.
  • Heart function tests such as blood pressure and angiograms.
  • Ultrasound imaging to check blood flow to the genitals.

 

Treatment methods

There are several ways to treat ED, and treatment depends on the cause of the issue. Treatment options include

Drug therapy:

  • Phosphodiesterase-5 inhibitors such as Viagra (Sildenafil), Cialis (Tadalafil), and Levitra (Vardenafil) help improve blood flow to the penis, and according to Harvard Medical School studies, these medications are the most common and effective.

Hormone therapy:

  • In cases of testosterone deficiency, your doctor may prescribe testosterone replacement therapy.

Psychotherapy:

  • In cases of ED caused by psychological causes such as anxiety or depression, a person may need to consult a psychiatrist or relationship specialist.

Cognitive behavioral therapy:

  • Focuses on changing negative thoughts about sex and increasing self-confidence.

Surgical treatment:

  • Penile implantation: In severe cases where other treatments don't work, an artificial penis may be implanted.
  • Injection of medications: Medications can be injected directly into the penis to stimulate erections.

 

Future research

Recent research suggests that gene therapy may be a future option in the treatment of ED. Several studies are developing treatments using stem cells to stimulate tissue growth and enhance blood flow to the genitals, and some initial trials have shown promising results, but research in these areas is still in its infancy.
